SUMMARY
Under the general supervision of the Administrator, the Case Managers help develop, implement, and evaluate individualized patient care plans as well as facilitate the discharge planning process by working with the resident, family, physician, nursing department, and social services to develop an appropriate and safe discharge plan for each resident.
EDUCATION/EXPERIENCE
To perform this job successfully, an individual must be able to fulfill each essential duty with a strong degree of professionalism. This position requires a detail-oriented professional with excellent communication skills. The requirements listed below are representative of the knowledge, skill and/or ability required.
- Minimum of H.S. degree or equivalent.
- Long-Term care experience working directly with individuals, families, and physicians to coordinate the discharge plan of patients.
- Current Licensure as a Registered Nurse or Licensed Vocational Nurse by the State of California.
- Must be certified in CPR.
