Overview

The Director for Primary and Urgent Care directs and is accountable for nursing practice, education and nursing services for Primary/Urgent Care. The director provides leadership for nursing and works with clinical teams to provide high-quality, cost-effective patient care. This person will also ensure full integration of nursing practice. This leader functions as a primary/urgent care nursing liaison across the continuum of care within the PHS Network and serves as the resource for the Primary /Urgent Care dyad for nursing and clinical processes.

Qualifications

  • Masters' Degree required in nursing or related field (Health Administration, Business, Public Health, or management).
  • If Masters is not in nursing, BSN required.
  • Master's degree must be obtained within five years.
  • State of New Mexico Nursing - RN License required.
  • Three to five years clinical experience required in a related area.

Responsibilities

  • Responsible for the clinical effectiveness and integration of all nursing/clinical activities within Primary/Urgent Care
  • With other members of the Primary/Urgent Care team, is responsible for the quality and sustainability of nursing/clinical services.
  • Actively participates in the strategic direction for Primary/Urgent Care clinical services in coordination with the Administrator Vice President and Medical Director of Primary/Urgent Care.
  • Maintain relationships with Lead Physicians, and Directors of Practice Operation (DPOs) in the coordination and planning of activities related to nursing services and disease management in Primary/Urgent Care
  • Directs the development, implementation, evaluation and revision of goals, objectives and standards of Primary/Urgent Care clinical services. Teaches, mentors and serves as a positive role model for nurses.
  • Develops and establishes policies and procedures related to nursing clinical practice within Primary/Urgent Care.
  • Coordinates nursing improvement initiatives for Primary/Urgent Care. Establishes quality monitoring systems with project teams to address issues.
  • Provides consultative services and expertise in regard to the development of the Primary/Urgent Care Nursing Care Model as part of the Lean transformation.
  • Coordinates Primary/Urgent Care Nursing Shared Governance model; supports and participates in PHS shared governance structure. Serves as communication resource with PHS Nursing Shared Governance.
  • Participates actively in recruitment, selection, orientation and evaluation of Primary/Urgent Care Clinic Managers.
  • Seeks opportunities to develop clinical teams by nurturing, advising and guiding peers and others. Monitors and evaluates the clinical team in collaboration with Primary/Urgent Care DPOs as needed
  • Effective collaboration with Presbyterian Health Plan to achieve Medicare Stars, HEDIS and HCC targets relative to primary care/Urgent Care
  • Result Driven: Able to execute and deliver on Quality Measures, Medicare Stars, HEDIS and HCC Targets
  • Effective collaboration with Population Health to align quality initiatives from PHP and Delivery system
  • Serves as a steward for Primary Care/Urgent Care Quality Scorecard measures
  • Facilitates the integration of evidenced based care into clinical practice and management.
  • In collaboration with the DPOs, ensures regulatory readiness, implements and maintains new compliance standards associated with all appropriate regulatory agencies
  • Leads and facilitates project teams to develop clinical redesign and implementation to meet future health care needs.
  • Collaborates with Clinic Managers to assess learning needs of clinical staff and development of Primary/Urgent Care orientation, training and educational programs.
  • Coordinates with Administrator Vice President and Medical Director of Primary/Urgent Care to develop, implement and evaluate clinical improvement initiatives
  • Performs other functions as required
