Pressley Ridge's Treatment Foster Care Program in Newark, DE has a current opening for a Treatment Coordinator. The position provides leadership in partnering with families and appropriate others in the community to assure that the therapeutic needs of a designated caseload of troubled children are adequately addressed. In each case the Treatment Coordinator will assure that the services delivered both to the target child/adolescent and to the family are individualized, appropriate to need, child centered, family focused, culturally competent, strengths based, and goal driven.

Essential Roles and Responsibilities

Supervision Training and Support

  • Assist and support in training activities for Treatment Foster Care (TFC) parents
  • Directly supervise all activities of treatment parents related to the treatment and care of TFC youth through regular home visits and phone calls
  • Provide in-home training as needed for TFC parents
  • Complete a formal TFC Parent Evaluation including creation and monitoring of a professional development plan for each family
  • Provide 24-hour emergency “on-call” service to TFC families according to designated schedule
  • Assist in the arrangement of respite services

Documentation and Compliance

  • Maintain documentation of in-home and other ongoing training activities
  • Prepare written summaries, Individual Service Plans and quarterly updates, discharge summaries, and program contract regarding youth and families
  • Ensure completion of and collect TFC parent documentation, including LODES, Medication Logs, Respite logs, copies of medical information, and any other required documentation

Treatment Planning and Service Delivery

  • Assess each child’s needs in all life domains and develop measurable and objective treatment plan goals
  • Provide ongoing advocacy and support for the child’s educational needs
  • Initiate and coordinate psychiatric, psychological, and other supportive therapeutic services when appropriate
  • Monitor the progress of the child’s goals and modify the treatment plan as needed
  • Provide overall leadership and coordination of the treatment team

Safety & Well Being

  • Assure that all policies, procedures and standards relating to child safety and well being are followed
  • Make regular contact with each youth to assess safety and well being
  • Assure that TFC parents follow procedures for child illness and provision of routine healthcare
  • Develop an individual safety plan for each child and assist other staff, in crisis intervention to protect youth, staff, and property when transporting youth, maintain safe driving procedures
  • Adhere to mandated child abuse reporting laws

Working with Biological Family

  • To maximize the likelihood of a positive relationship between TFC youth and their biological families
  • Assess and identify the needs of the birth family to achieve their family service plan goals
  • Empower birth families through systems education and advocacy training
  • Involve the biological family in all aspects of treatment planning including regular updates on youth’s progress
  • Coordinate regular contact between the child and his/her biological family as appropriate

Communication & Community Resources

  • assure continued communication between the programs, the child’s family, relevant community agency and case aides
  • Develop each youth’s treatment team including formal and informal supports as identified by the youth, family, and guardian
  • Maintain regular contact and open lines of communication with youth’s treatment team and any other significant community persons
  • Represent agency regarding court involvement of the youth
  • Link youth and family to appropriate pro-social activities that foster success and healthy development
  • Respond to all complaints/concerns from TFC parents in a timely supportive manner

Permanency Planning

  • assure the development, implementation and monitoring of effective treatment services in transitional living for youth targeted
  • Confirm youth have completed an independent living assessment for life skills
  • Submit assessment to the appropriate individuals for scoring
  • Supervise any program aides that are assigned to the youth to assist in the implementation of the treatment and service plan
  • Work in collaboration with other service providers to develop and implement intervention procedures in the homes working toward achievement of the permanency plan
  • Coordinate all community resources pertaining to discharge placement
  • Arrange aftercare follow up if indicated

Professional Development

  • Actively participate in individual and/or group supervision and staff meetings
  • Attend and participate in all required and other professional trainings
  • Independently seek out resources and learning opportunities to foster ongoing professional growth

    Working Conditions

    • Physical Demands. Requires vision, speech, and hearing and a moderate to high level of physical activity including; sitting, standing, walking lifting and carrying as required. Must be able to meet the demands and complete Therapeutic Crisis Intervention (TCI).
    • Environmental Factors. Indoors/Outdoors: school, community, residence, office
    • Working Hours. A non-traditional work schedule as defined by service needs and crisis “on call”

This position requires a LBSW .

Clearances. State Police/FBI clearance per state regulation; Child and Adult Abuse Registry clearance

Valid drivers license and current insurance

Experience

Required
  • 2 year(s): supervised experience with children and/or families

Education

Required
  • Bachelors or better in Human Services or related field

Licenses & Certifications

Required
  • LBSW
