JOB SUMMARY
The Family Peer is a person with lived experience trained to provide support to other family members who are caring for a person with a mental illness. Activities will include assisting family members in navigating different health care systems, advocating for families in different settings such as school, role modeling behaviors, linkage with other social supports, and providing encouragement and hope.
DUTIES AND RESPONSIBILITIES
- Engages and communicates effectively with youth and their families/legal guardians, and all team members.
- Provide emotional support for the family on their parenting journey to reduce isolation, feelings of stigma, blame and hopelessness.
- Assists the family to implement strategies recommended by clinicians (e.g., medication management, behavior support, crisis plan) and talk to clinicians about their comfort with these plans.
- Provides outreach, information, and support to consumers families to empower them to make decisions, connect them with resources and be a warm line for them to call for information.
- Participates in the development/reviews of individualized plans for youth and their families.
- Supports the efforts of families in caring for and strengthening their youths mental, and physical health, development, and well-being. Develops healthy, supportive relationships with youth and their families.
- Maintains regular contact with each youths family based on the individualized plan.
- Models strength-based interactions by highlighting positive encounters.
- Provides transportation to families (as required) to and from appointments/support activities.
- Prepares families for meetings and accompanies them when needed e.g., school, medical, mental health, substance abuse, court, etc.
- Advocates for youth and family within a variety of settings, including, but not limited to school, juvenile court, medical and mental health agencies, and other community-based service providers.
- Works closely with families of consumers to provide appropriate support and access to resources such as family support groups.
- Maintains a working knowledge of Center policies, procedures, and licensure regulations governing Presteras service delivery.
- Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.
- Utilizes technology provided to facilitate services and/or perform job efficiently and effectively.
- Meets training requirements as established, including but not limited to, CPR-First Aid and crisis intervention.
- Participates in other functional work by participating in related projects, as applicable and directed by Center leadership.
SUPERVISORY RESPONSIBILITIES
No direct supervisory responsibilities are required.
QUALIFICATIONS AND REQUIREMENTS
EXPERIENCE, KNOWLEDGE, AND SKILL REQUIREMENTS:
- Demonstrate lived experience as the parent or primary caregiver who has navigated multiple child-serving systems on behalf of their child(ren) with social, emotional, developmental, health and/or behavioral healthcare needs.
- Valid drivers license required.
- Commitment to supporting and working with families.
- Knowledge of community resources preferred.
- Ability to learn, navigate, and work daily in the EHR system effectively.
- Excellent interpersonal skills and ability to develop trusting relationships and handle sensitive and confidential situations.
- Possess good written and verbal communication skills in conjunction with good time management and organizational skills.
- Has shown ability to be creative and think outside the box in situations.
- Must be able to lift to 25 lbs.
EDUCATION AND TRAINING REQUIREMENTS:
- High School Diploma or General Educational Development Certificate required.
- Maintains First Aid/CPR certification.
- Maintains current crisis prevention/intervention training.
