Pride Health is hiring a Case Manager for our client in Madison, WI!
Schedule: 7:30pm-4pm
Responsibilities:
- Reviews all patient related social service consults, providing appropriate follow up to aid in safe discharge planning
- Assess all assigned patients and integrates family information with nurse care managers to determine potential, actual, and next level of care
- Collaborates with physicians, nursing, therapy and other multidisciplinary teams, payers, and agencies to eliminate barriers to efficient delivery of care in the appropriate setting.
- Documents clinical activity in the medical record and supports and coaches accurate clinical documentation by other treatment team members
- Coordinates and facilitates access to services and patient care progression using best practice interventions that will produce favorable patient outcomes.
Qualifications:
- Minimum of BSW from an accredited college/university with three years social work experience
- MSW from an accredited college/university with two years social work experience preferred
- Ability to adapt in fast paced environment
