Pride Health is hiring a Nurse Practitioner or Physician Assistant in Wake/Cumberland County, NC! (contract)
Schedule: Monday-Friday (8am-5pm))
Contract: 7 month contract
Pay Rate: $67-69/hour
Specific Duties:
- Provide specialized care to high-risk health plan members
- Work as part of an interdisciplinary team
- Conduct comprehensive assessments
- Manage acute and chronic medical and behavioral conditions
- Ensure accurate documentation, including ICD-10 coding
- Communicate and collaborate with the care team
- Facilitate advanced illness and care planning discussions
- Offer counseling and education to patients and caregivers

Must have qualifications/experience:
- Nurse Practitioner or Physician Assistant
- NP Certified in one of the following (FNP, ANP, GNP)
- PA: Board Certified by National Commision on Certification of Physician Assistants
- Access to transportation
- 1 year of experience (LTC, SNF, Geriatric preferred)
