This job posting requires travel in Robeson, Bladen, Lee and Hoke counties.
Must have a Master Degree and a valid Counseling License.
Provides individual and family therapy for each youth served by the team
Designates the appropriate team staff such as specialized clinical expertise is applied as clinically indicated for each child
Provides and coordinates the assessment and reassessment of the recipient’s clinical needs
Provides clinical expertise and guidance to the IIH team members in the team’s interventions with the recipient
Provides the clinical supervision plan is required for all IIH team members exclusive of the Team Leader
Investigates case situations and presents information to professionals such as the Psychiatrist and Clinical Psychologist and other members of the mental health team, on the consumer’s family and social background pertinent to diagnosis and treatment.
Maintains documentation in client medical records as required
Monitors and reviews services in all settings that includes the review of the documentation of all direct care staff/contract providers
Be able to assist the consumer and family to diffuse current crisis situation and reduce recurrence
Ensure linkage to needed community services and resources
Provide self-help and living skills training for youth
Provide parenting skills training to help the family build skills for coping with the youth’s disorder
Monitor and manage the presenting psychiatric and/or addiction symptoms
Work with caregivers in the implementation of home based behavior supports
Responsible for coordinating the initial assessment and developing the PCP and for 27G.0104 and according to licensure and certification requirements of the appropriate discipline will provide supervision to Paraprofessional, Associate Professional, and Qualified Professional.
The team will provide services to a family ratio that shall not exceed one to eight (1 to 8) for each three person team.
The intensive in home services at the beginning of treatment and decrease over time as the youth and family’s coping skills develop
Deliver structured and face to face services to provide support and guidance in all areas of functional domains
Accepts training to enhance skills and knowledge in order to have and maintain a working knowledge in relation to job, therapeutic intervention and treatment modalities
Maintains daily documentation
Attends treatment team meetings and other meetings as required
Responds to crisis situations when requested
Adheres to and follows all policies and procedures of this agency
General office hours are from 8:00 am to 5:00 pm, Monday to Friday.
Intensive In-Home service requires that the staff be available 24 hours per day, 7 days per week, and 365 days per year.
There are no shift rotations, but employees will be expected to adjust schedule as needed to: (1) accommodate client who is unable to keep an appointment during routine office hours, or who may require emergency services; (2) make community presentations; and (3) represent this agency at civic or professional function
