Responsibilities:
- Understands and adheres to established Company policies and procedures.
- Supervises field staff, clinical nursing and other therapeutic services furnished.
- Provides oversight for patient and field staff assignments.
- Coordinates care and referrals processes.
- Assesses patients’ needs, and ensures a plan of care is developed, implemented, and updated.
- Understands and promotes principles of continuous performance improvement.
- Responds to the planning and consultative needs of field staff.
- Assists the Director of Clinical Services in the preparation and implementation of policies and procedures which meet Medicare, Medicaid, accrediting bodies, state, and local laws.
- Assists the HR Team in recruiting, hiring, evaluating, and terminating nursing personnel. Assists the HR Team in instituting, interpreting, and documenting disciplinary action.
- Serves as a role model for all field staff by setting an example of high standards in professional care, job performance, cooperation, conduct, and attire.
- Acts as a scribe to assist field clinicians with accurate and timely documentation during start of care charting.
- Observes confidentiality and safeguards all patient related information.
- Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regard to time involved.
- Investigates and reports to the Director of Clinical Services any problem relating to patient care and/or employee well-being.
- Attends pertinent continuing education programs/training other than routine clinical services and shares information with clinical staff.
- Assists the Director of Clinical Services in the development of process improvement activities with appropriate data collection, aggregation, analysis, taking action and reporting of results according to Company’s QAPI plan.
- Assists Director of Clinical Services in conducting patient care conferences, field staff meetings and maintains documentation.
- Reviews patient medical records for compliance with federal, state, accrediting bodies, local and Agency policies and guidelines.
- Ensures adherence to Department of Public Health, Medicare and Medicaid standards across all clinical teams by remaining up to date on relevant standards, conducting field staff training in conjunction with the Director of Clinical Services, developing protocols and procedures as needed, and performing intermittent/regular reviews and audits.
- Trains field and office staff across various teams as needed in accordance with home health care regulations.
- Works with nurses across the Department of Clinical Services to ensure regular clinical competency verification of nursing staff.
- Conducts analyses for clinical adverse events and near miss events; develops action plans to reduce risk of recurrence; follows the progress of these action plans in conjunction with the Director of Clinical Services.
- Identifies areas of potential risk and assists the Director of Clinical Services in developing adequate measures within the Company to minimize risk of adverse events or negative quality outcomes
- Monitors equipment for appropriate use and takes steps to keep misuse to a minimum.
- Ensures that all necessary supplies and equipment are available
- Must be able to perform nursing functions in times of staff shortage.
- Must be able to act as the Director of Clinical Services sign in the Director’s absence.
- The Clinical Manager is available during all operating hours and participates in all activities relevant to professional services furnished, including the development of qualifications and appraisal of the field staff.
- Other duties as assigned by the Director of Clinical Services.
Qualifications:
- Graduate of an approved school of professional nursing and currently licensed in the state of Ohio as a Registered Nurse, required.
- 3-5 (three to five) years of health care experience, required.
- Minimum 2 (two) years’ experience in home health care, preferred.
- Minimum 1 (one) years of managerial experience preferred.
- Ability to exercise initiative and independent judgment, required.
- Ability to work with individuals, to enlist cooperation of many people to perform/achieve a common goal, required.
- Experience in providing healthcare related training, preferred.
- Experience in working with clinical databases and applications (Devero, TigerConnect, etc), preferred.
- Basic skills in Microsoft Office (Word, Excel, PowerPoint) required.
- Good communication skills required.
- Excellent ability to interact effectively by active listening, asking questions and explaining concepts required.
- Ability to prioritize tasks and projects within short time frames, multitask and meet deadlines required
- Punctuality and attendance required.
