Key Role Details :
- Travel Enthusiasts : This role centers around EXTENSIVE DRIVING TO VARIOUS NURSING FACILITY LOCATIONS, SO A RELIABLE VEHICLE IS A MUST!!
- Tech-Savvy : Comfortable with technology and patient outreach, you'll be on-site at different Nursing Facility locations.
- Patient Advocate : If you're applying to be a PrimoCare Health Liaison, you should be compassionate, patient, and dedicated to building trust with patients.
Duties and Responsibilities :
As a key member of our dynamic team, you'll play a vital role in enhancing our patients' healthcare experience.
In this role, you will :
- Connect with patients who are initiating remote patient monitoring programs.
- Serve as the primary point of contact for patients enrolled with PrimoCare, guided by their healthcare providers.
- Assist patients in completing assessments using iPads.
- Regularly engage with your assigned patients to ensure they stay on track with their treatment plan and remain within the date range set by their provider.
- Schedule consistent monthly check-ins with your patients.
- Monitor patient data to ensure their well-being.
- Empower and educate patients by sharing valuable health information.
- Collaborate with the PrimoCare team through informative meetings to stay updated on the latest developments.
Minimum Qualifications :
To thrive in this role, you should bring the following qualifications to the table :
- A valid CNA or Medical Assistant License with a minimum of 2 years of practical experience.
- A robust sense of ambition, self-motivation, and self-discipline.
- Exceptional communication skills, both written and verbal.
- A strong grasp of the fundamentals of chronic health conditions, along with the operation of relevant medical devices, such as glucose monitors, blood pressure cuffs, pulse oximeters, spirometers, scales, and more.
- Proficiency with technology, including the ability to navigate computer platforms and familiarity with Excel.
- Comfort with patient outreach and engagement.
- Experience in interactions with patients and clinical staff.
- Highly organized, efficient, detail-oriented, and adept at multitasking.
- A team player who thrives in a collaborative environment.
Preferred Additional Qualifications :

- Experience in telemarketing, sales, or customer service.

Expectations and Compensation :
- Work hours from Monday to Friday, between 9 : 00 AM and 5 : 30 PM PST.
