What are the responsibilities and job description for the Program Coordinator position at Pristine Health Services Inc.?

Program Coordinator will work closely with the Program Manager and Direct Support Professionals (current and new) to help promote independence and provide community engagement opportunities for persons with Intellectual and Developmental Disabilities. Coordinators will directly supervise, train, and coach current and new DSPs with a person-centered approach.

QUALIFICATIONS:

  • Bachelor’s Degree in related discipline preferred
  • Two years’ experience working with individuals with intellectual and developmental disabilities and/or supervisory experience preferred.
  • Obtain CMT certification within first 90 days, as needed.
  • Maintain a valid driver’s license and automobile insurance with a driving record of less than 5 points.
  • Use of personal vehicle to transport individuals as needed.
  • Ability to relate well to staff and board members of different races, ethnicities, genders, sexual orientations, socioeconomic backgrounds, and religions.
  • Background Check required.

RESPONSIBILITIES

Personnel/Supervision:

  • Assist manager in training and onboarding of DSPs; Provide on-boarding to new staff using a variety of orientation strategies, including the use of mentors and peer-to-peer feedback
  • Identify additional resources for DSPs, or for changes to service delivery, both within and outside of agency, and advocate for these resources with managers.
  • Provide direct supervision to DSPs
  • Complete performance reviews for DSPs
  • Use technology such as cell phone, email, text messaging, and video chats effectively in supervisory tasks
  • Teach, model, and coach staff in the most effective approaches to achieve the direct support competencies
  • When delegating responsibilities, provide instructions and resources to DSPs to ensure successful completion of tasks.
  • Actively participate in the personal professional development plan by identifying occupational interest, strengths, options, and opportunities
  • Promote the ongoing competency-based training and development of DSPs
  • Maintain communication with program manager regarding staffing concerns
  • Understand role and responsibilities of DSPs and be able to fill in as needed
  • Ensure all staff meet all training requirements
  • Enforce and abide by all agency and program rules, policies, and procedures.

Department Administration:

  • Implement and monitor department/agency policies and procedures.
  • Coordinate with all other departments and programs (e.g. Individual Advocate, Transportation, Nursing Services, Admissions, etc.) as needed
  • Guide the development and implementation of programming and person-centered activities for the individuals

Individual Service Responsibilities:

  • Maintain thorough understanding of person-centered thinking and individualized services
  • Ensure intervention strategies are developed and implemented in accordance with the individual plan, behavior modification plan, and/or the needs of the individual
  • Actively observe for signs of neglect, maltreatment, or violation of rights and take immediate action to remedy the situation
  • Encourage individuals to be as engaged in all aspects of his/her daily life
  • Support and encourage community-based activities and services, including but not limited to education, recreation, and employment opportunities.
  • Identify individual preferences and needs and ensure service planning and implementation are designed to meet those needs.
  • Participate in the development of engaging, person-centered activities that focus on increasing independence and skill development
  • Using positive behavior support strategies, develop, implement, and monitor support plans designed to teach self-management and promotes wellness, recovery and crisis prevention with a person being supported engaged in challenging or risky behavior.
  • Assist with planning and coordination of activities and outings based on individual need/ plan.
  • Administer medications in accordance with protocols as assigned
  • Identify crisis or potential crisis situations and intervene to facilitate best possible outcome; maintain calm demeanor throughout therapeutic interventions.
  • Participate in the individual and collaboration team meetings as needed
  • Maintain, protect, and share pertinent knowledge of individuals as appropriate
  • Make recommendations for individuals to be referred to different service areas based on performance, preferences, attendance, quality of work and needs

Communication and Documentation:

  • Maintain all program operations and individual’s records in compliance with state and federal regulations as directed by the program.
  • Complete and/or approve incident reports as needed within 24 hours of occurrence.
  • Maintain communications with individual’s team, document contacts as appropriate.
  • Notify supervisor and team members of changes of individual’s goals, preferences, employment status, etc. in a timely manner.
  • Complete/submit all personnel/ agency documentation within required timeframes.
  • Oversee daily documentation of DSP

Agency/Team Cohesion:

  • Actively participate in intra- and inter-departmental team meetings.
  • Facilitate teamwork and positive interactions among teams and between DSPs by managing conflict and providing counseling and support to DSPs as needed in all work sites, especially for DSPs in remote work settings
  • Coordinate and facilitate staff meetings, ensuring a sense of trust and openness and encourage group participation and ownership
  • Develop and maintain mechanisms and provide support for agency and departmental cohesiveness.
  • Encourage cooperation from all staff members with other departments within the agency.
  • Attend all pertinent team/agency meetings, trainings, and activities.
  • Attend supervision meetings with supervisor

  • Provide proper notice of lateness or absence; follow procedures for requesting time off.
  • Maintain positive, professional work behavior with supervisory staff, co-workers, individuals, families, funders, employers, and the community
  • Convey a professional image when representing Pristine Health Services Inc. at the agency and in the community.
  • Prepare for and attend meetings in a timely manner.
  • Successfully complete all required training on time.
  • Follow all departmental and agency policies and procedures
  • Maintain all emergency and safety requirements

Job Type: Full-time

Pay: $19.00 per hour
