Home Health Physical Therapist needed for Sun Prairie/Beaver Dam , Ideal schedule M-F 8-5. Home Health experience not required, training and advancement opportunities offered!
As a home health clinician with PRN HH&T you will visit patients in ALFs and in their homes. You will have the opportunity to observe the patients in their home environment. Many Home Health patients don’t see a lot of people, so you’ll also be an important part of their day!
About Us: We are a family-owned company, based out of Appleton and serving 20 counties in central east WI. We offer Skilled Nursing, Physical Therapy, Occupational Therapy, and Speech Therapy. We believe flexibility is important to the success of our employees, since you schedule directly with your patients, you can have a late start, an early start, and even a chunk of time off in the middle of the day!

You are a therapist who:
· Values building connections with their patients.
· Likes working independently while backed by a collaborative team.
· Has great written and oral communication
· Holds or can get a current License/certification in Wisconsin
Job Duties Include:
· Evaluating and developing individualized patient treatment plans
· Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
· Instruct Physical Therapist Assistants in treatment procedures to be utilized and provide supervision.
· Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
