Mobile Wound Care Nurse Practitioner

Department: Wound Care

Position Summary:

The Wound Care NP works as a member of the team to provide wound care to patients within skilled nursing, long-term care, transitional care, assisted living, memory care, private homes with or without home health. This position is responsible for providing wound management for wounds collaboratively with the facility wound nurse/nursing team. The Wound Care NP is responsible in creating and maintaining a plan that results in successful wound care outcomes. The Wound Care NP will do on site visits with patients and provide consultative phone calls and/or telehealth visits with nursing staff and Providers with questions on continuum of wound care.

Classification: Up to full time.

Education/ Qualifications:

  • Graduate of an accredited Nurse Practitioner
  • Licensed by the New Mexico Board of Nursing and in good standing for NP
  • DEA License
  • BLS and CPR certified
  • Ability to react calmly and effectively in emergency situations
  • Ability to interpret clearly and establish/maintain effective working relationships with patients, medical staff, and facilities
  • Work well independently in addition to being skilled in interdisciplinary collaboration
  • Is a self-starter, motivated, and demonstrates positive attitude
  • Valid and unrestricted driver’s license

Essential Functions:

  • Ability to market wound care services to designated facilities, build caseload, and form and maintain relationships with facility staff
  • Develop and manage an individualized wound care treatment plan for optimal healing outcomes or chronic maintenance
  • Collaborates closely with facility wound nurse and nursing staff on wound management practices
  • Knowledge of up-to-date topical wound care products and therapies, including ordering, application, and management of the products on wounds and ostomies, also including negative pressure wound therapy
  • Provides hands on wound care to patients including debridement of wounds by enzymatic, sharp, or autolytic methods when appropriate
  • Orders, reviews, interprets, and integrates diagnostic studies and laboratory findings for initiating or modifying the treatment plan
  • Collaborate with the patient’s primary care provider when comorbidities are contributing to or affecting the healing of a wound to determine a comprehensive plan
  • Update the primary care provider on the status and management plan of the wound as appropriate
  • Prescribes medications and/or durable medical equipment to assist in healing of wounds
  • Provides the required billing information for services provided by the Wound Care NP and maintains the documentation necessary for those services billed
  • Reviews patient prior medical records and maintains their current medical record related to the wound diagnosis and contributing comorbidities
  • Patient chart encounters will be closed according to the standard of care
  • Assist with the identification of wound care quality measures, and provide data and documentation for tracking of quality measures
  • Work with partners to determine opportunities for education of nurses with appropriate identification of wound types, prevention of wounds, and nursing care of wounds as needed
  • Develop wound care education plans in collaboration with partners as needed
  • Instructs patient and family regarding wound treatments and management plan for optimal wound outcomes
  • Triages telephone calls and provides consultation
  • Participates in professional development activities and maintains professional affiliations.
  • Completes required number of continuing medical education (CME) hours for license and certification
  • Attends required meetings or committees as requested
  • Maintains patient confidentiality
  • Performs other duties as determined collaboratively with team and administrators that are within the scope of practice of the Wound Care NP
  • Maintains a copy of all professional records including current licensure, certification, continuing education, and other appropriate records to practice
  • Patient-centric approach

  • Possess dexterity and ability to become proficient with medical instruments and equipment as required
  • Travel: 100%

Physical/Mental Demands and Work Environment:

  • Involves exposure to communicable diseases, medical prescriptions, toxic substances, and other conditions
  • Requires full range of body motion to get in and out of car, as travel is required
  • Frequent standing, walking, and sitting required for extended periods of time
  • Requires visual and hearing acuity
  • Able to lift a minimum of 50 pounds
