Job Details: Job Details and Responsibilities: Effectively and positively represents the company and its pharmacy partners while executing the full scope of services offered to patients, caregivers, hospital case managers, physicians, nursing agencies and other clinicians. Facilitates patients’ successful clinical coordination of care from the prescribing provider into the home setting. Provides and uses clinical expertise and skills to assist clinicians, their staff, and patients with: the identification of appropriate candidates for home infusion or home health care; the assessment of a patients’ or caregivers’ ability to provide care; patient education and training regarding therapy and supplies, as well as clarifying expectations of care in the home; medication connection at the hospital / facility to expedite the discharge into the home setting. Evaluates current and future clinical and service needs for patients and prescribing clinicians. As appropriate or required, coordinates or conducts in-service or continuing education programs for clinicians and their support staff. Collaborates with prescribing providers and affiliated staff (i.e. hospital case managers, home health agencies, physician offices) and pharmacy partner staff to facilitate the coordination of care for patients of partner pharmacies. Coordinates and communicates all discharge planning activities for new patients and prescribing providers, including, but not limited to: Intake Clinical Operations: Pharmacy, or Nursing coordination Appropriately and accurately documents coordination of care activity in compliance with professional, company, and partner standards, including: Home infusion or nursing services; All communication with patients, caregivers, physicians, physician staff, and partner; Patient or caregiver education and training. Consistently communicates with, and seeks feedback from, home health partners to improve and enhance the discharge process for patients transitioning to the home. Collaborates, coordinates and communicates with company administrative staff to provide information needed for effective back-office administration (e.g., authorization processes, claims accuracy) related to new patient on-boarding and the seamless transition to home infusion services. Complies with all company and partner policies and directives, including successful completion of all partner orientation and compliance training (both new hire and ongoing) Communicates ongoing process improvement collaborations between the partner and company. Performs other duties as directed. Necessary Skills Strong communication skills, specialty therapy experience, ability to multi-task, detail multiple specialty therapies and disease state focus planning. Demonstrates thorough knowledge of core therapy regimes (e.g., specialty infusions, antibiotics, TPN/Enteral Nutrition, Intropic Agents) and qualifying criteria by payor. Payor knowledge, along with the ability to navigate Medicare, Medicaid and commercial insurance plans to assist with the intake benefit and authorization process. Strong willingness to stay abreast of developments in field of infusion therapy. Ability to accurately and professionally document all records necessary to patient transition of care. Ability to communicate and work professionally and compassionately with varied audiences including patients, caregivers, patient families, physicians, nurses, hospital staff, and others involved in a patient’s care. Full Time (this is not a work from home position); Ongoing, consistent face to face communication with patients, caregivers, clinicians, and partner staff are required to ensure the best possible experience in transitioning infusion patients to a home setting. Qualifications Associates or Bachelors degree from an accredited college or school of nursing (if applicable to the job requirements; Hold an active, unencumbered nursing license by the state Board of Nursing or Nursing Regulatory Body/Hold and maintain current BLS certification). Specialty infusion experience Home health care experience Must possess a valid and current driver’s license and auto insurance. Must maintain a professional image and abide by company dress code.
