Position Summary:
We are seeking a full-time Case Manager to join our Work and Gain Education and Employment Skills (WAGEES) Reentry Program, providing vital support to justice-involved participants (18 – 24 years old) as they transition from incarceration or other justice system involvement back into the community. Reporting to the Program Director, the Case Manager will play a key role in guiding participants toward economic self-sufficiency, financial stability, and the development of essential life skills.
The Case Manager will assess program eligibility, provide tailored case management, and work closely with participants to address their individual needs. By promoting active engagement in program activities and training, the Case Manager will guide participants in achieving personal growth and securing access to high-growth, livable wage employment opportunities. The Case Manager will also facilitate access to postsecondary education in alignment with participants’ goals.
About the WAGEES Program:
The WAGEES Program assists justice-involved youth and young adults and prepares them for employment through education, job training, paid work experiences, mentorship, conflict resolution, leadership development, and case management. In addition to job training and paid work experiences, this program provides support with obtaining unsubsidized employment and/or postsecondary education.
Key Responsibilities
- Inform applicants and participants of program expectations, benefits, requirements, and procedures.
- Assist applicants and participants in completing documents upon which eligibility determinations are based. Verify accuracy, completeness, consistency, and timely submission of program eligibility documents.
- Provide referrals to appropriate community resources and services based on participant needs and requests.
- Enter and retrieve numerical and narrative data using an automated case management system, maintaining accurate and up-to-date records in compliance with program regulations.
- Conduct formal and informal assessments to identify participant strengths, needs, and barriers to self-sufficiency.
- Build rapport with participants and actively involve them in the decision-making process.
- Offer ongoing mentoring, support, and coaching to participants, assisting them to achieve their personal, professional, and educational goals in alignment with the program.
- Conduct community outreach and participate in local events to support participant recruitment efforts and expand referral network.
- Lead workshops and group sessions aimed at enhancing participant professional development and achieving program outcomes.
- Collaborate with other staff members and partner organizations to address and resolve barriers to participant engagement.
- Prioritize workload and manage tasks to ensure compliance with program regulations and deadlines.
- Maintain professional standards, including adherence to Project Hope’s core values and employee policies.
- Perform other related tasks as assigned.
Required Qualifications:
- Bachelor’s degree in Social Work, Sociology, Public Administration, or a related field, 2 years of case management experience in reentry or community violence intervention programs.
- Excellent organizational skills and attention to detail.
- Strong interpersonal and communication skills, with the ability to build rapport and maintain professional relationships with participants, colleagues, and external partners.
- Ability to prioritize workload, manage multiple tasks, and meet deadlines in a fast-paced environment.
- Valid driver’s license, auto insurance, and reliable transportation required for community outreach and off-site meetings.
- Proficiency in Microsoft Office and Google Workspace

Preferred Qualifications:
- 3 years of reentry case management experience in community-based organization (CBO) settings
- Knowledge of Pomona community resources, social service programs, and reentry support systems.
- Experience using a case management database.
- Training in trauma-informed care, restorative practices, motivational interviewing, and/or case management best practices highly desirable.
