Home Health Caregiver

Prolife Caregiver LLC in Whitman, MA is looking for one home health caregiver to join our strong team. Our ideal candidate is a self-starter, punctual, and reliable.

General Summary

Prolife Caregivers is an employee of Prolife Caregiver LLC, a Home Healthcare Agency. This position provides daily personal care services, light housekeeping and other related services to our clients and members according to the Individual Service Plan (ISP) created by Registered Nurse (RN) & Case Manager. 

Responsibilities

• Provides daily personal care and homemaking services to our clients & members according to the ISP developed by the RN and Case Manager consistent with Prolife Caregiver LLC policies and procedures, vendor agency policies and procedures, and in compliance with MassHealth guidelines.
• Observes and informs RN and I Case Manager on matters and issues related to the well-being of each client or member.
• Notifies management of any concerns/issues with the provision of service, including absent visits.
• Develops positive working relationships with our clients, members and staff.
• Performs all duties consistent with the contracted vendor agency job description.
• Complete required trainings and continuing education opportunities provided by CareAcademy.

Qualifications

• Successful completion & documentation of the MA mandated training course
• Maintains appropriate certification in good standing by successful completion and documentation of annual re-certification trainings
• Demonstrates competency in personal care/homemaking and related required skills to perform tasks outlined on the Individual Service Plan
• Experience working with adults that are elderly and/or persons with disabilities in a community setting or equivalent at the professional discretion of the vendor agency
• Strong communication skills and ability to complete important documents
• Bilingual is a plus

Evaluation and Review

• The Caregiver reports the RN Supervisor and Case Manager
• Mandatory quarterly evaluations with Pro-life Caregiver LLC management team
