This position is responsible for the delivery of skilled nursing care and supportive services within the scope of the nurse's education and experience. Key responsibilities involve coordinating care delivery to ensure that clients' needs are met in accordance with professional standards of practice through physician orders, ProMedica policies and procedures, and federal, state and local guidelines.

Educational Requirements
Currently registered/licensed in the state, and in good standing with the Board.
Position Requirements
Qualifications
Good communication skills, both verbal and written.
Demonstrated problem solving and decision-making abilities.
Ability to develop and maintain rapport with the patient and family.
Experience
A minimum of six months (institutional**) nursing experience within the past five (three *) years. Evidence of successful completion of a refresher program can be accepted in lieu of this recent experience requirement.

Travel
100%; necessary daily.
Working Conditions
Client’s home/living facilities.
Exposure to disagreeable conditions including odors, infection, illness, and physical contact from disruptive patients.
Job Specific Details:
General Responsibilities
Provides services in accordance with organizational policies and procedures and regulations that define scope of practice.
Prepares clinical and progress notes documenting outcomes of interventions.
Assists the RN and/or physician in performing specialized duties related to end-of-life care.
Assists the RN in carrying out the plan of care.
Prepares equipment and materials for treatment adhering to aseptic technique as required.
Assists the patient in learning appropriate self-care techniques.
Assesses patient/family response to care.
Ensures communication of information to appropriate team members.
