Providence Care brings Hospital at Home services to our community. We are inviting Registered Nurses to join our growing Home Health or Palliative Care nursing divisions in the Chester, SC and surrounding areas by providing full time, Case Management services to patients in their homes, assisted living and long term care communities.

A Brief Summary of Your Outcomes:

• Coordinate and administer care with a team of professionals, including medical clinical director, medical social workers and other passionate caregivers.

• Collaborate with Physician for new orders, to advise of needs and/or changes.

• Provide education to patients and families on disease processes and palliation.

• Attends and actively participates in weekly team meetings.

• Provides rehabilitation or palliation care and coordinates medical needs for clients.

Qualifications:

• Must have a RN license eligible in the state you are applying

• Willingness to be flexible and work some on-call hours potentially as needed

• Reliable transportation and valid drivers license

• Passion for helping others

• Wound care experience is a plus

• Willingness to travel and attend to patients in their home setting
