What are the responsibilities and job description for the At-Home Physician position at Providence?
As a Physician with Providence Medical Group at Home, you’ll join a skilled interdisciplinary team dedicated to providing primary care to patients in a home-based, community setting. The group is seeking a board-certified/board-eligible Family Medicine or Internal Medicine Physician full-time at 1.0 FTE. In this role, you will provide primary care with an emphasis on management of chronic conditions, disease prevention, and symptom management to patients that are not best served in a primary care clinic. - Full-time at 1.0 FTE at 5 days per week
- Monday to Friday, daytime hours
- Outpatient, primary care in a home-based, community setting
- Work within an interdisciplinary team including pharmacy, care management, nursing staff
- Ideal candidates will have previous clinical experience working in primary care, home health, geriatrics, and/or palliative medicine
