Description:
Care Management Associate
The Care Management Assistant (CMA) collaborates with the patient/family, the acute muRequltidisciplinary team, and post-acute providers to implement the patients discharge plan. The CMA also proactively escalates risks and barriers to discharge to the Case Manager; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning.

Required qualifications:
- Completed an LVN program from an accredited school -or- 10 years or more care management experience -or- Completed Bachelors degree
- 2 years of experience in health care setting (acute or post-acute)
- 2 years of LVN experience (if LVN)
- 2 years of experience in health care case management discharge planning
