Providence is calling a Clinical Social Worker (LCSW) (Per diem/Day Shift) to Providence Holy Cross Medical Center in Mission Hills, CA.

We are seeking a Clinical Social Worker (LCSW) who will provide biopsychosocial assessment, treatment planning, and implementation of social work services to patients and families along the continuum of care. The Clinical Social Worker utilizes his/her knowledge and clinical skill sets to address needs of vulnerable populations and in complex discharge planning situations. In addition to addressing barriers to coping with illness or injury, the LCSW has a specialized role with regard to assessment and intervention of abuse, suicide risk, and other mental health issues. The LCSW demonstrates proficiency in delivering care to assigned age specific populations (e.g., infants, pediatrics, adolescents, adults, and geriatrics).
Having been licensed by the State of California to provide psychotherapy services, the LCSW provides clinical leadership in the use of psychotherapeutic techniques and interventions for individuals, families, groups, and crisis situations. Psychotherapy as defined by the State of California Board of Behavioral Sciences is "..the use of psychosocial methods within a professional relationship, to assist the person or persons to achieve a better psychosocial adaptation, to acquire greater human realization of psychosocial potential and adaptation, to modify internal and external conditions which affect individuals, groups or communities in respect to behavior, emotions, and thinking, in respect to their intrapersonal and interpersonal processes." The LCSW functions as a clinical leader and consultant within the healthcare setting, including special projects, committees, and other initiatives as appropriate.
In this position you will have the following responsibilities:
- Adhere to departmental, unit, and hospital policies and procedures, including but not limited to HIPAA, patient/customer safety, environment of care, and infection control
- Interview patients and family members to perform biopsychosocial assessment, including but not limited to assessing for abuse, suicide and/or homicide risk, and other safety risk factors that may impact on life safety
- Based on findings from biopsychosocial assessment, formulates a treatment plan in the context of the healthcare setting
- Implement treatment plan that includes assisting patients and families to understand and participate in the medical plan of care and utilize appropriate community resources
- Facilitate communication between patient and family through patient-family counseling, and with the healthcare team through family conferences, interdisciplinary care planning conferences, and case consultation
- Participate in the development of safe and appropriate discharge and post-hospital care plans
- Provide counseling, social support, and assistance in crisis situations and when conflicts arise with service providers
- Collaborate with other hospital programs and community agencies with which patients and families are involved
- Coordinate services with appropriate hospital and community resources
- Prepare and maintain required documentation and reports.Attend meeting required in the performance of their duties, and other meetings or committees as assigned
- Enhance professional growth and development through participation in educational programs, current literature, in service meetings, workshops and supervision/case consultation
- Demonstrate an awareness of therapeutic use of self and the impact in the provision of applied psychotherapy of a non-medical nature
- Institute appropriate treatment planning and intentional use of treatment modalities
- Actively participate in clinical and administrative consultation with clinical supervisor as it applies to patient care and organizational intervention/services
- Is proficient in advocating for sound social work principles, methods and values in the context of patient care, as well as organizational and community conduct
- As an advanced practitioner, may provide clinical leadership and mentoring of colleagues, interns and other related disciplines
- As assigned, may represent the social work perspective in planning and policy development
Qualifications:
Required qualifications for this position include:
- Master's Degree in Social Work, from a CSWE university
- Licensed Clinical Social Work by California Board of Behavioral Science
Preferred qualifications for this position include:
- One (1) year clinical experience in a healthcare setting
