Discharge Planner - Part-Time .6 FTE Days(8 Hours shifts, every other weekends)

Plans and coordinates activities related to transition planning. Develops and implements a plan of care designed to meet the patient's post hospitalization needs while ensuring continuity of care. Responsible for the collaboration with medical staff, patient, family, community agencies, or facilities involved in patient's care, and the health care team by assessing, facilitating, planning and advocating for health needs on an individual basis resulting in quality, cost-effective outcomes.

Required Qualifications:

  • Master's Degree - Social Work or Counseling.

  • Washington Clinical Independent Social Worker Associate License upon hire. Or,

  • Washington Advanced Social Worker Associate License upon hire. Or,

  • Washington Mental Health Counselor Associate License upon hire. Or,

  • Washington Agency Affiliated Counselor Registration upon hire.

  • Experience in discharge planning, care coordination, or community resource navigation, in a clinical or community setting.

Preferred Qualifications:

  • Experience in discharge planning for medical and or surgical patients in an inpatient or outpatient setting.

  • Experience in an acute hospital setting doing discharge planning is preferred.
