About the Role
We are seeking a highly skilled and experienced Home Health Clinical Manager to join our team at Providence. As a key member of our Home and Community Care (HCC) service line, you will play a critical role in coordinating multidisciplinary home health services for our clinical team.
Key Responsibilities
- Coordinate home health services for our clinical team, ensuring seamless communication and collaboration among team members.
- Act as a liaison between Access Data Entry, Scheduling, Quality, and clinical staff, providing essential information and tools to maintain best practices.
- Provide leadership and guidance to our clinical team, promoting a culture of patient-focused, whole-person care.
- Collaborate with our finance team to compile data and reports, utilizing workflow and financial planning information to inform decision-making.
Requirements
- Bachelor's Degree in a healthcare-related field.
- Washington Registered Nurse License upon hire.
- National Provider BLS - American Heart Association within 30 days of hire.
- 3 years of experience in nursing or rehab.
Preferred Qualifications
- Case Management experience.
- Supervisory experience in Home Health.
