Providence is calling a Mental Health Specialist - Mental Health Adolescent Child (0.0 FTE, Variable) to Providence Willamette Falls Medical Center in Oregon City, OR.
We are seeking a Mental Health Specialist - Mental Health Adolescent Child who will work with other members of inpatient unit staff to provide patient care in a variety of job duties, and under the direct supervision of the RN, or the Mental Health Professional Therapist, or the inpatient unit Manager or Director. Provides hospital level and focused patient care, support, and milieu management for patients with behavioral health conditions and illness. Under the direction of an RN or other designated clinical staff, observes and reports on patient conditions. Responsible for and/or supports the daily and various therapeutic activities care of the patients. Collaborates with the multi-disciplinary treatment team in the initial developing and modification of treatment care plans both during the admission process, and during the in-patient hospitalization.
In this position you will have the following responsibilities:
- Provide milieu management of patients using a relational approach under the guidance and supervision of the RN, or other mental health professionals on the unit
- During the admissions process, may participate in the gathering of information from families as outlined by departmental admission procedures
- Give input and recommendations to the initial care plan including practical and therapeutic approaches that may benefit the patient
- Provide feedback and information either directly to the RN and other mental health professional team members regarding observed behaviors of patients, or charts the information directly into Epic including all activities whether they are within or outside the norm of expectations on the unit
- Work directly with the RN's and other mental health professionals to give valuable input and recommendations to the care and treatment plans of the patients throughout their hospitalization
- Act in a collaborative team environment to maintain the effective and proper functioning of the unit by monitoring patient condition and activity
- Utilize various de-escalation techniques as needed
- Survey and monitor the unit at all times to ensure security and safety are maintained
- Provide life support in emergency situations until code team arrives
- Maintain high level of awareness of the condition of unit milieu and problems which could endanger others including observations of patients in relation to their disabilities, age and medical diagnoses
- Conduct and/or co-facilitate with the MHP both group and individual therapy in the areas of recreational, skill building, mindfulness, or other therapeutic activities as assigned on the unit's daily schedule in a manner that is consistent with maintaining professional boundaries
- Provide timely and appropriate therapeutic support for group or individual activities that are in alignment with the MHS role and meet the agreed upon needs of the milieu
- Be responsible for the development and facilitation of leisure and exercise activities for patients of all ages and abilities
- Perform direct observations of patients with suicidal or homicidal thoughts; and observes family or collateral involvement as directed
- Report observations to the RN or other mental health professional, or charts the information directly into Epic
- Support activities and actions to improve collaboration and liaison with other departments referring agencies and outside providers
- Assist discharge planning process of the patient by making and completing phone contacts with providers and others
- Contribute to high quality unit functioning by maintaining professional standards and ethics and strict confidentiality of information
- Accompany patients off of unit to procedures within the hospital
- Report immediately any changes in patient condition, physical or behavioral to the RN assigned to the patient
- Perform routine and assigned duties to ensure the proper maintenance and functioning of the unit for quality assurance, regulatory guidelines, and as needed, examples of this include inventory of patient belongings during the initial admission process as well as checklist of patient belongings and related documentation upon discharge
- During the discharge process, may support the physical warm hand-off of the patient to their family or the next provider, this is dependent upon the relationship of the Mental Health Specialist to the individual patient
- Travel to the patient's family or next provider may be required
- Coordination of Care:
- Create strength-based, collaborative care planning with the patient and their family/support systems
- Develop and maintain effective working relationships with various departments, community health, social service agencies, payors and other partners
- Advocate for support appropriate to the age, biopsychosocial needs, cultural, spiritual, and health literacy of the patient. Identify barriers to continuing care and planning of next level of care
- Function within the existing care team model
- Collaborate with the inter/multi-disciplinary team facilitating care conferences for complex patient care coordination
- Advocacy:
- Act as an advocate for the needs and interests of the patient and their family/support system by supporting self-determination and autonomy
- Advocate for navigation of complex healthcare systems, various payers, community services, education, system changes, etc.
- Adhere to mandatory reporting of abuse, neglect, and/or exploitation
Qualifications:
Required qualifications for this position includes:
- Bachelor's Degree in Social Science fields (Social Work, Sociology, Psychology or other related field)
- American Heart Association (AHA) - Basic Life Support (BLS) Provider certification required before hire
- Re-certification every two years
- Two (2) years in an acute or residential inpatient psychiatric facility in a similar role
- Applicable internship or practicum experience may be applied towards years of experience
- Knowledge of and experience with crisis intervention and assessment techniques
- Understanding of family systems
Preferred qualification for this position include:
- Experience in a child or adolescent psychiatric environment
