Providence is calling a Social Worker ( Social Work Srvs , Day Shift, 0.0 FTE, on-call) to Providence Mount Carmel Hospital in Colville, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.
This is an on-call position to cover with ill caregivers or PTO/vacation. At this time, shift coverage expectancy is about 1 shift per month, possibly every-other month.

In this position you will:
- Assess patients for care management services after a referral has been made based on the Admission screening and referrals for potential patient issues:
- Ability to perform psychological, social, environmental, spiritual, financial, and functional assessments
- Ability to work independently in an unstructured environment
- Skill in prioritizing, time management and organization
- Appropriately identify actual and potential care issues within scope of practice:
- Critical thinking, analysis, and problem solving skills
- Ability to follow through on responsibilities and commitments
- Develop and manage a coordinated plan of care to meet care managed patient and/or family needs within scope of practice. Recognize circumstances requiring nursing intervention:
- Creative thinking and vision regarding possibilities for care options based on patient preferences.
- Identify and acts upon barriers to wellness
- Determine patient resources, availability of financial means/insurance benefits
- Ensure that patients are placed on appropriate clinical pathway and protocols, individualizes patient care needs through consultation with the multidisciplinary team:
- Strong verbal and written communication skills
- Knowledge of clinical pathways and other protocols and guidelines
- Monitor the patient/family progress towards achieving optimal functional status:
- Clinical expertise including psychosocial assessments, and co-morbid conditions
- Organize and link appropriate services within the hospital and community to deliver coordinated care to patient and family; including palliative support care services within the facility and communities. Facilitate coordination of relations between patient/family and these services. Evaluate post-acute care environment for accessibility and adaptive needs:
- Ability to identify support needs of the patient/family representatives and refer to other consultants
- Skills in helping patient/families deal with end-of-life decisions and major life changes
- Ability to provide emotional support
- Plan, lead, participate in and follow through with multidisciplinary team meetings as needed for improving coordination of care services. Assess the teaching needs of the patient and family members based on patient goals and objectives. Assess effectiveness of the Palliative/Supportive Care program; Review Palliative care screening tool results, palliative/supportive care assessments and consultations results. Share findings with appropriate interdisciplinary team members and recommends alternative interventions:
- Ability to analyze
- Knowledge of TQM process
- Mathematics and basic statistics expertise
Qualifications:
Required qualifications for this:
- Bachelor's Degree in Social work Or applicable job experience
Preferred qualifications for this position include:
- Master’s degree in Social Work (MSW) or equivalent education/experience
