Overview: We are seeking a dedicated and experienced Population Health Manager to join our team in Wichita, Kansas. The Population Health Manager will play a crucial role in developing and implementing strategies to improve the health outcomes of our member population. This individual will oversee all aspects of population health management, including wellness programs, chronic disease management, preventive care initiatives, and community outreach efforts.
Responsibilities:
- Develop and implement population health strategies to improve health outcomes and reduce healthcare costs.
- Design and oversee wellness programs aimed at promoting healthy behaviors and preventing illness.
- Coordinate chronic disease management initiatives to support members with ongoing health conditions.
- Analyze population health data to identify trends, gaps in care, and opportunities for improvement.
- Collaborate with internal teams and external stakeholders to integrate population health initiatives into care delivery models.
- Lead community outreach efforts to engage members and promote health education and preventive services.
- Monitor program outcomes and adjust strategies as needed to achieve desired results.
- Stay informed about industry trends, best practices, and regulatory requirements related to population health management.
Qualifications:
- Current licensure as a Registered Nurse (RN) in the state of Kansas.
- Minimum of 3-5 years of experience in healthcare, with a focus on population health management or care coordination.
- Strong understanding of healthcare delivery systems, quality improvement principles, and population health concepts.
- Excellent analytical skills and ability to interpret and present data effectively.
