Description
EPISODE ASSESSMENT COORDINATOR, RN with home health experience and OASIS and ICD10 certifications required. This is a remote position and the RN that fills this job will live in either GA, NC, SC, or the Jacksonville / Tallahassee, FL area.

Responsibilities
- Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
- Complete all documentation and records in a timely and accurate manner
- Follow established policies and procedures as related to the scope of practice
- Maintain continuity of patient care in a home care setting
Qualifications
LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
- Proof of COVID-19 vaccination or approved exemption required by date of hire
