JOB OVERVIEW:

The National Nurse-Led Care Consortium (NNCC) is a national 501(c)(3) nonprofit dedicated to advancing nurse-led models of care through policy, technical assistance and consultation, and innovative programming.

The Public Health Project Coordinator (Social Services) is responsible for one or more public health projects for NNCC. These projects may be funded by various sources, including federal and state grants and private corporations. The Coordinator is responsible for the day-to-day activities of the programs and supports reporting responsibilities. This position will support families participating in the Family Advocacy and Integrated Resources (FAIR) Project who receive legal services and financial assistance to improve family and child health and wellbeing. FAIR Project families are connected to diverse services by working collaboratively with a team of attorneys, paralegals, and public health professionals. The Coordinator will provide direct assistance to participants to help them navigate administrative agencies, including assisting with completing applications, monitoring the status of participants’ applications, conducting the appropriate follow-up, and providing case management. The Coordinator will provide linkages to care and community-based social services and supports and develop key relationships and partnerships with community resources and organizations. The Coordinator will collaborate with others and demonstrate positive communication skills in a variety of settings. The Coordinator will periodically share role responsibilities and resources at conferences and invited presentations. The Coordinator reports to the Family Resources Manager and other senior staff regarding quality improvement initiatives across service lines.

NNCC is a nonprofit public health organization working to strengthen community health through quality, compassionate, and collaborative nurse-led care. NNCC is the largest provider of nurse home visiting programs in Philadelphia, serving approximately 800 families annually. In 2022 NNCC launched the FAIR Project in partnership with Public Health Management Corporation and Health, Education and Legal Assistance Project: A Medical-Legal Partnership. The goal of the FAIR Project is to improve the health and wellness of children and families by ensuring that pregnant people and parents have the opportunity to raise their children in safe, healthy homes, with uninterrupted health insurance coverage and other benefits.

RESPONSIBILITIES:

  • Manage referrals from the FAIR Project Intake Team to conduct initial assessments related to participant eligibility for public benefits, housing resources, and other social services and community-based resources.
  • Identify community-based resources and social services that would benefit participants and families.
  • Provide direct navigation assistance to participants and families, including assisting with applications for benefits and services, following up with agencies, and monitoring the status of participants’ applications.
  • Respond to requests received from internal and external partners in a timely manner.
  • Build trusting relationships with participants and their families.
  • Develop partnerships and MOUs with community-based social services agencies and other organizations that will benefit participants and families.
  • Coordinate administrative, programmatic, and grant management activities of public health projects.
  • Act as a liaison with funding agencies, project partners, and other critical stakeholders.
  • Work with supervisor to meet all reporting requirements.
  • Adhere to program policies and procedures.
  • Work with supervisor to create and manage service provider contracts to meet contractual deliverables.
  • Maintain documentation of activities, referrals, and outcomes in a secure, electronic case management system.
  • Participate in weekly and as-needed team meetings with project staff.
  • Coordinate community engagement strategies with others that include site visits with service providers.
  • Function as expert liaison with staff and community groups.
  • Contribute to trainings for home visitors and community partners on various topics.
  • Contribute to evaluation activities and report development.
  • Participate in record audits and quality improvement strategies to contribute to NNCC strategic planning initiatives.

WORK CONDITIONS:

  • Flexible schedule, including some early evenings and weekends for special events (onsite or offsite)
  • Light to moderate lifting and transport of program/client materials
  • Use of public transit or private transportation as needed
  • Local or regional travel as required by funders or senior staff

REQUIREMENTS:

Education:

  • Associate’s degree in a relevant field required
  • Bachelor’s degree or current enrollment in a bachelor’s degree program preferred
  • Collegiate social work or public health studies

Experience:

  • Minimum of 2 years of relevant professional experience in public health, nursing programs, maternal-child health, integrated health systems, or a related field
  • 1 year of experience helping families navigate community-based resources and social services preferred
  • Familiarity with public health home visitation models and preventive health initiatives
  • Knowledge of Philadelphia neighborhoods and the city at large
  • Lived experience navigating public benefits preferred

Other:

  • Spanish language skills preferred
  • Current clearances including FBI, PA child abuse, PA criminal history, and others required by PHMC (or willingness to obtain clearances during the credentialing process)
  • Ability to work onsite or offsite per assignments/commitments
  • Some Saturday availability for outreach and community events throughout the year

SALARY GRADE: 18
