Responsibilities
Tasks will vary based on the patients’ specific needs, with ongoing responsibilities including:
- Assuming responsibility for evening, and holiday on-call duties, as schedule, in keeping with hospice policies and procedures
- Assuming primary responsibility for total care census, including assessing, planning, implementing and evaluating all phases of the nursing process
- Communicating with attending physicians and other community resources to report changes and coordinate optimal care/use of resources for patients and families
- Communicating with the Clinical Coordinator to ensure appropriate dissemination of information and follow through on the part of families and caregivers
- Attending weekly interdisciplinary team meetings to maintain up-to-date patient records, troubleshoot problems, sets plans and action goals and work in tandem to adjust care plans, as needed
- Obtaining data on physical, psychological, social and spiritual factors that may influence patient/family health status, and incorporating those findings into care plans
- Monitoring physical care of patients and educating family, caregivers and volunteers in proper, approved techniques
- Responding to medical or emotional crises and/or patient deaths, and make home visits to provide direct care, as appropriate
- Providing appropriate counseling at patients’ times of death, and lending immediate support to families and caregivers
- Carrying out additional assignments, as instructed
Qualifications
- RN license to practice in Pennsylvania
- Must be flexible, exhibit a keen ability to work independently, execute sound judgment and successfully troubleshoot for a host of medical and non-medical situations
- At least year of varied nursing experience in an acute, oncology, home health and/or hospice setting
- Valid driver’s license and reliable mode of transportation
