Duties and Responsibilities
- Participate in community planning related to the interests of the facility and the services and needs of the resident and family.
- Participate in admission assessments and discharge planning, development and implementation of social care plans and resident assessments.
- Interview resident/families as necessary and in a private setting.
*Education
- Must possess, as a minimum, a Bachelors Degree from an approved school of Social Work.
Specific Requirements
- Must have experience in a skilled nursing care facility, or other related medical facility.
Experience
- Must be able to read, write, speak, and understand the English language.
- Must possess the ability to make independent decisions when circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
- Must a willingness to work harmoniously withothers.
Job Type: Part-time
Pay: $32.00 - $35.00 per hour
Expected hours: 12 – 16 per week
