Job Opportunity: In-Person Case Manager (Home Health Agency)

Position: In-Person Case Manager Location: Palmetto Bay, FL (33157) Type: Full-Time (Monday – Friday)

Responsibilities: As an In-Person Case Manager, you will play a crucial role in coordinating and managing patient care. Your responsibilities will include:
· Reviewing clinical assessments to evaluate patients’ needs and develop personalized care plans.
· Collaborating with interdisciplinary teams, including nurses, therapists, and physicians, to ensure comprehensive care.
· Providing education and support to patients and their families.
· Monitoring patient progress and adjusting care plans as needed.
· Documenting patient interactions accurately and maintaining up-to-date records.
· Advocating for patients’ well-being and ensuring their safety.
· Monitoring and assisting patient and caregiver calls related to the services rendered.
· Assisting and working in tandem with the Intake Coordinator to ensure a positive onboarding and transition of care to patients.
· Monitoring and recording incident reports for tracking of patient incidents (falls, infections, hospitalizations)
· Case conferencing with Director of Nursing and clinical team regarding patient outcomes, care coordination, service delivery
Qualifications: To excel in this role, you should possess the following qualifications:
Home Health Care Experience: At least 1 year of experience in home health care or a related field.
Health Care Knowledge: Familiarity with health care regulations, best practices, and patient care standards.
Computer Skills: Proficiency in using electronic health records (EHR) systems and other relevant software. May be trained as long as basic computer skills knowledge.
Bilingual Skills: Fluent in both English and Spanish.
Compassion: A genuine desire to make a positive impact on patients’ lives.
