Social Worker (LSW)

The Bay at Burlington is hiring for a passionate Social Worker (LSW) to join our 123-Bed Skilled Nursing facility. As a Social Worker, you will work closely with our residents to ensure quality care is provided to improve the lives of our residents.

Duties and Responsibilities: 

• Maintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needs
• Identifying individual social and emotional needs
• Developing and maintaining individualized care plans
• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay
• Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Qualifications/Requirements:

• Degree in Social Worker
• Licensed Social Worker (LSW)
• At least one year experience in Social Work in Long Term Care or similar settings
• Experience working with a diverse aging population
