Job Summary:

Quality Care Solutions is currently hiring for a Psychosocial Rehabilitation Direct Care Specialist position in Rocky Mount, NC providing services to clients with mental or emotional disorders by teaching them to function in their own community. We are a facility and community-based agency designed to provide professional, quality services to individuals and families in a collaborative effort to inspire a positive and healthy future through connections of advocacy, resources, and support. We strive to help men and women recover from mental health and substance use disorders. We are an agency committed to quality gender-responsive, trauma-informed care to clients with substance use and co-occurring disorders. The Psychosocial Rehabilitation Specialist is responsible for assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients' needs in a manner that reflects the agencies mission.

The Psychosocial Rehabilitation (PSR) Specialist must be able to plan and provide rehabilitative services for individuals with mental illness. Responsibilities include developing treatment plans, providing individual and group instruction and completing documentation of services provided. PSR Specialists will be responsible for scheduling community and center base activities that are based on the member’s needs. The PSR Specialist is also responsible for maintaining the safety and well-being of all members while receiving services on-site or in the community.

Duties

Maintain a client caseload and productivity by maintaining regular contact with members to monitor and assess progress

Identify client’s strengths, needs, and abilities and address barriers to meeting client goals

Use a strengths-based and trauma-informed approach to work collaboratively with members and internal and external team members to help support persons served in their treatment and recovery

Utilize components of CBT to assist members with their recovery

Assist members with community integration, transportation, and life skill development

Goal setting and treatment planning

Linkage with community resources and referrals

Provide supportive services that encourage independence

Crisis intervention

Document all activities in a timely and accurate manner

Remain in compliance with internal and external policies, procedures, regulations, and standards, including all of the agency’s policies and procedures and Quality Assurance Plan, and State Licensing standards.

Participate in all assigned meetings, staff development, and training as required Core Competencies:

  • Demonstrate ability to work with multi-disciplinary adult recovery teams.
  • Knowledge of Trillium behavioral health system.
  • Ability to engage peers in recovery plan.
  • Good communication skills.
  • Uses good judgment as it relates to PSR clients.
  • Knowledge of confidentiality, ethics and boundary issues related to peer support

Job Requirements:

  • Hold a Bachelor's degree in a human services field from an accredited university and one year of relevant experience with the target population or
  • Hold an Associate's degree in a human services field from an accredited university and two years of relevant experience with the target population or
  • Hold a high school diploma or equivalent (GED) and three years of relevant experience with the target population

Job Type: Part to Full-time

Pay: $13.00 - $15.00 per hour
