This position is for our Quality Life Services location in Markleysburg, PA.

RESPONSIBILITIES

·        Certified Registered Nurse Practitioner (CRNP) is responsible for resident health and care within our skilled nursing/personal care facility

·        Maintains compliance with PPHP Model of Care, Completion of Comprehensive Health Record (CHR), and reviewing appropriate HEDIS measures

·        Assigning patient risk per PPHP policy

·        Document any Transfer of Care (TOC)

·        Review all testing, labs, and consults

·        Complete walking rounds and communicate regarding resident concerns

·        Report and log hospital admissions and any unplanned transfers

·        Coordinate and complete employee physicals

REQUIREMENTS

·         Completion of an accredited Nurse Practitioner program

·        Current and unrestricted NP license in PA
