Adolescent Case Manager

JOB SUMMARY
The Adolescent Case Manager at Rainbow Health is responsible for connecting individuals to support services, such as mental health care, transitional housing, health insurance, and transportation assistance, to help address high rates of homelessness and other unmet social needs among LGBTQ and BIPOC young adults, ages 14-24. Our case management program works from client-centered, strength-based, harm-reduction, and trauma-informed perspectives; comfort working from these perspectives is required. It also requires being able to work with medical and social service providers throughout the community and documenting interactions with clients, their caregivers, and providers in a timely manner.

ESSENTIAL FUNCTIONS
  • Engaging with clients one-on-one to develop rapport and build relationships.
  • Maintaining a caseload and managing client needs to respond to emerging needs and engage in proactive client work.
  • Completing formal and informal assessments with individual clients to identify client strengths and barriers to meeting needs, emphasizing needs connected to managing adolescents identifying as LGBTQ or as part of the BIPOC community.
  • Participating in outreach activities on behalf of Rainbow Health's Free to Be program through community and organizational events.
  • Developing action plans in partnership with clearly defined goals and steps the client and a case manager will take to achieve identified goals.
  • Providing resources and connections between young adult clients and other community partners or providers.
  • Meeting with clients in provider offices and community settings, including community centers, public libraries, client homes, etc.
  • Completing narrative written documentation of client interactions using professional and objective language.
  • Supporting team members in work efforts through client consultation, coverage, referral education, etc.
  • Other duties as needed to best support the agency and our clients.

QUALIFICATIONS
  • High school diploma.
  • Background in social services, mental health, or social work.
  • Bilingual candidates are strongly preferred.
  • Individuals with lived experience or a passion towards our target populations (Gay/Bi/MSM/Queer, Transgender, African American, multi-racial communities that are disproportionately impacted by HIV, HIV positive people, and those engaging in risk behaviors for HIV in individual and group settings) are encouraged to apply.
WORK ENVIRONMENT
  • Work is currently in a hybrid format, splitting time between the community and working from home. The person hired for this position must be comfortable meeting people within the community, participating in community events, and attending in-person meetings.
  • Employees working in person with clients or out of our offices must show proof of vaccination and attest to their vaccination status upon hire. Employees who are not vaccinated will be required to show a negative COVID-19 PCR test result once a week if working with clients or in our offices. Religious and medical exemptions or reasonable accommodations will be considered upon request as required by applicable law

This position is part of a bargaining unit represented by SEIU Healthcare Minnesota & Iowa. All wages, benefits, and other terms and conditions of employment will be established through negotiations with the union.
