GENERAL FUNCTION: The person in this position provides high level of supervision to 25 mentally ill

persons experiencing a behavioral health crisis in the CSU/MMCWM/Respite Program . These patients

have high acuity, have decompensated in symptoms and require supervision for safety. Monitors and

makes written documentation of clients' behavior. Provides direct services and administers medications

to high volume of clients in the Crisis program.

SUPERVISORY RESPONSIBILITY: None

QUALIFICATIONS; KNOWLEDGE AND REQUIRED TRAINING: The person occupying this position must be

trained as a Licensed Practical Nurse and be licensed as a Licensed Practical Nurse in the State of

Tennessee.

DUTIES AND RESPONSIBILITIES:

1. To assess and treat the needs and problems of a high volume of individual clients experiencing a

behavioral health crisis in the CSU/Respite/MMCWM and CAC programs as needed. Complete

admission process and documentation on 25 patients daily.

2. To participate in the development of treatment plans for 25 clients experiencing a behavioral

health crisis in the CSU/Respite/MMCWM and CAC programs as needed.

3. To provide individual and group psychosocial training to 25 clients experiencing a behavioral

health crisis in the CSU/Respite/MMCWM and CAC programs as needed.

4. To make recommendations regarding 25 client's treatment who are experiencing a behavioral

health crisis in the CSU/Respite/MMCWM and CAC programs as needed.

5. To evaluate the progress of 25 client's treatment who are experiencing a behavioral health

crisis in the CSU/Respite/MMCWM and CAC programs as needed.

6. To complete a mental status exam that includes the ongoing nursing evaluation of a client's

current condition for 25 clients who are experiencing a behavioral health crisis in the

CSU/Respite/MMCWM and CAC programs as needed.

7. To evaluate the effectiveness of medication in managing the symptoms of the client for 25

clients who are experiencing a behavioral health crisis in the CSU/Respite/MMCWM and CAC

programs as needed.

8. To evaluate the medication for adverse effects, adverse reactions, drug-drug interactions, drug-

food interactions, and drug-disease interactions.

9. Based on the evaluation performed and with the purpose of optimizing the pharmacotherapy,

referral to the prescribing professional as to the proper drug selection, dosage adjustment and

treatment of adverse effects, adverse reactions and/or interactions for 25 clients who are

experiencing a behavioral health crisis in the CSU/Respite/MMCWM and CAC programs as

needed.

10. Immediately alert the prescribing professional to any of the following findings: serious adverse

effects/reactions/interactions of medications, medical conditions and/or medications which

may require alteration of current medication, exhibiting a serious emotional crisis, exacerbation

of illness or symptoms, suicidal or homicidal ideation, and development of a serious medical

complication for 25 clients who are experiencing a behavioral health crisis in the

CSU/Respite/MMCWM and CAC programs as needed.

11. Supervise Respite clients who are experiencing a behavioral health crisis in the self-

administration of medication.

12. To administer intramuscular medicines as needed including emergency medicines to individuals

at risk of self-harm or harm to others in the CSU/Respite/MMCWM and CAC programs as

needed.

13. To complete documentation and record keeping as soon as possible following medication

administration to 25 clients who are experiencing a behavioral health crisis in the

CSU/Respite/MMCWM and CAC programs as needed.

14. To educate 25 clients who are experiencing a behavioral health crisis in the

CSU/Respite/MMCWM and CAC programs as needed about their mental illness, how their

medications act to control their illness/disease state, the importance of complying with the

medication and coping with the adverse effects, adverse reactions and interactions which may

accompany the medication.

15. To educate 25 clients who are experiencing a behavioral health crisis in the

CSU/Respite/MMCWM and CAC programs as needed client and/or family regarding key

components of their care to maintain compliance and stabilization.

16. To communicate with the prescribing professional, members of the mental health treatment

team, and other health care providers including the Primary Care Physician in an effort to

coordinate efficient and effective care for 25 clients who are experiencing a behavioral health

crisis in the CSU/Respite/MMCWM and CAC programs as needed.

17. To coordinate daily visits with the prescribing professional for 25 clients who are experiencing

a behavioral health crisis in the CSU/Respite/MMCWM and CAC programs as needed.

18. To participate in discharge planning for 25 clients who are experiencing a behavioral health

crisis in the CSU/Respite/MMCWM and CAC programs as needed.

19. To complete documentation within required Center timeframes for 25 clients who are

experiencing a behavioral health crisis in the CSU/Respite/MMCWM and CAC programs as

needed.

20. Monitors vital signs, blood sugar levels, blood alcohol levels, administer pregnancy tests,

administer urine drug screens, and draws bloodwork for labs for all applicable patients in

CSU/Respite/MMCWM and on CAC as needed.

21. This position administers medication per standing orders or other prescribing provider order to

all applicibable patients on CSU/Respite/MMCWM and on CAC as needed.

22. Immediately alert the on-call/prescribing professional to any of the following findings: serious

adverse effects/reactions/interactions of medications, medical conditions and/or development

of a serious medical complication, need for isolation, need for emergency meds.

23. Be trained and provide CPI techniques when need to de-escalate clients and prevent self-harm

or harm to others.

24. To complete documentation and record keeping as soon as possible following medication

administration, blood alcohol checks, urine drug screens, pregnancy tests, receipt of labwork

results, communication with a prescribing provider, or communication with another facility.

25. To work as a member of a multi-disciplinary team and communicate with the prescribing

professional, members of the mental health treatment team, and other health care providers

including the Nurse or Physician at a receiving facility in an effort to coordinate efficient and

effective care for the client.

26. Complete a Nurse-to-Nurse report with other agencies as part of a referral for services process.

27. Observe and monitor client behavior, provide supervision to all Crisis clients.

28. Assist manager in monitoring and maintaining supplies, equipment and appliances. This

includes maintaining inventory.

29. Monitoring and maintaining work area cleanliness.

30. Other duties as assigned.

PERSONAL RELATIONSHIPS: In order to appropriately accomplish his/her duties, the person

occupying this position will be required to maintain effective relationships with a wide variety of clinical

staff people, mental health consumers, and other professionals.
