JOB SUMMARY:
To manage the process of patient referrals to Ranken Jordan in collaboration and support by the Referral Liaison MD. To collaborate with referring clinicians from OSH to evaluate patients referred to Ranken Jordan Pediatric Bridge Hospital. Evaluating and determining appropriateness, admission criteria, plan of care, and medical readiness of patients referred to Ranken Jordan. Leads the Referral RN and Intake Coordinator to optimize patient referrals leading up to admission.
DUTIES AND RESPONSIBILITIES:
1) Collaborates with and supports the Referral Liaison MD within Referral Development and engagement within the referring medical community.
2) Manages the Referral RN and the Intake Coordinator to optimize the referral process including from the time of referral to time of admission.
- Provides leadership within the organization in areas affecting patient throughput, from pre-referral to post discharge, including in engaging in Census and Marketing efforts including data assessment.
- Directs and assists the Referral RN/Intake Coordinator to obtain necessary information from referring hospitals as well as communicating and preparing Ranken Jordan to effectively/safely admit referred patients.
3) Assess and evaluate referred patients prior to admission to Ranken Jordan to determine their appropriateness and medical stability.
- Develops strong relationships with referring medical community especially those involved with the care of children with medical complexity (CMC).
- Develops plan of care for referred patients prior to admission.
- Provides education and support to referring sources to assist in the continuum of care for patients transitioning to post-acute care with eventual goal of discharge to home.
- Conducts visits to referring hospitals to evaluate patients and communicate with their clinicians and caregivers.
- Manage communications between Ranken Jordan clinicians and the referring medical community.
4) Responsible for developing and communicating day-to-day plans for hospital admissions.

QUALIFICATIONS AND SKILLS REQUIRED:
- Current valid license issued by the State of Missouri as a registered professional nurse, completion of Masters of Science in Nursing and certification by the Missouri State Board of Nursing as an Advanced Practice Nurse.
- Current BLS and PALs Certifications.
- 2-3 years as a nurse practitioner required, Hospitalist experience is preferred.
- Ability to project a professional image and be assertive while building trust with regional healthcare community contacts.
- Strong interpersonal and communication skills required. Exceptional written and verbal communication skills and the ability to work well with the public, staff, and volunteers.
- Excellent planning and organizational skills, strong personal and professional integrity, team orientation, good judgment, and attention to detail.

PHYSICAL DEMANDS:
These include stooping, kneeling, crouching, reaching, grasping, pulling, pushing, standing, carrying and lifting of light loads (up to 35 lb.) as necessary.
