JOB DESCRIPTION: Job Coach
POSITION LEVEL: Non-Exempt
QUALIFICATIONS: High School diploma or its equivalent. Must have a car and a flexible schedule. Must be authorized for coverage under agency insurance if operating agency vehicle. Must have good driving record, valid Wisconsin driver's license, and/or required level or personal auto insurance. Must have at least 3-5 years job experience with good computer and communication skills.
REPORTING SUPERVISOR: Director of Human Services & Program Development
GENERAL DUTIES: Provide training, supervision, and support services to ensure the success of participants working in jobs in the community as well as those preparing for community employment.
*Denotes essential job function.
SPECIFIC DUTIES:
*Provide on-site supervision and quality checks at job sites.
*Provide training and guidance in job-related skills to individuals employed in the community and through classroom instruction for those preparing for community employment. (e.g. mobility, grooming/hygiene, attendance/punctuality, transportation, communication with supervisor, co-worker relationships, etc.)
*Identify barriers to successful employment, then troubleshoot and develop strategies to compensate for the barriers. These barriers may occur on or off the work site.
*Communicate observations, concerns, changes, and progress on goals to Case Manager and Employment Services Coordinator.
Act as primary contact for those RCS participants who are in dual programming and ensure all employment related information is shared between parties to include RCS Case Managers, participants, and employers.
*Complete documentation and timekeeping records detailing the services provided to each participant in accordance with funding agency specifications. This requires proficient use of a computer.
*Establish and maintain positive working relationships with employers, funding agencies, and community partners.
*Develop natural supports and promote independence and growth for the participant.
*Identify additional tasks within the job site that could be performed by current or future participants.
Participate in job-related training and staff meetings.
*Assist as needed, and as directed by Director of Human Services & Program Development, with the assignment of hours and planning of Job Coach schedules.
Assume additional duties as directed by the Director of Human Services & Program Development.
