Case Manager - Forensic Assertive Community Treatment
Do you have a passion to help individuals with behavioral health diagnoses and/or addiction on their journey to maintain psychiatric stability and sobriety? Are you willing to work with clients to develop and implement a comprehensive treatment plan that addresses the clients’ daily living needs with the support of other team members?
In this role, you will work directly with clients to develop treatment plans that will maximize independent functioning. You will be part of an integrated team which will support the client in their journey to stability and wellness, providing regular contact and updating the team weekly; while complying with documented requirements of the federal, state, local board and Agency standards.
Qualifications
Minimum qualifications for the position include:
- Minimum high school diploma and three year of direct case management experience, or in lieu a bachelor’s degree in a relatable field
- Good interpersonal, written and verbal communication skills
- Understand mental illness, symptoms and impact on behaviors; understand concept of recovery/resilience
- Be comfortable transporting clients (valid driver license, vehicle, and insurance a must)
