Description
ReDiscover is a nonprofit community mental health agency that offers a full spectrum of programs and services for people whose lives have been affected by serious mental illness and/or substance use. ReDiscover helps men, women, and children who have limited income, no insurance or who are under-insured.
We started out in Lee's Summit, MO as a group of volunteers in the basement of a local church. We incorporated in 1969. Over the years, the community has known our agency and our unique programs by several different names, including Research Mental Health, North Star, and Common Ground. In 2003, we permanently changed our agency name to ReDiscover.
POSITION OVERVIEW
Under the general direction of the School & Community Services Manager, the QMHP provides biopsychosocial assessment and case management services as appropriate for children and youth who are ReDiscover clients in accordance with the standards of clinical practice established by the State of Missouri and the policies and procedures of Rediscover.
Duties include but are not limited to the following:
- Assess children and families accurately and completely using all the diagnostic tools required.
- Develop treatment plans that match the needs of the child/family/community and works toward meeting goals of the treatment plan.
- Follows through with all tasks and interventions that are identified by all parties involved
- Integrates service provision to all parties to address the needs of the client.
- Demonstrates clinically sound judgment and seeks appropriate supervision and/or training for difficult or hard to serve clients
- Maintains clear concise communication that is timely and accurate with all parties to the service provision.
- Meets with clients/families within the timeframes established by the program.
- Coordinates with all other professionals involved in the case from a team approach.
- Individualize services to address the direct needs of the client/family.
- Maintains healthy, appropriate professional boundaries with all parties involved with the clients/families/resources served.
- Makes appropriate referrals to internal/outside agencies to meet the needs of the client/family.
- Develops a working relationship/partnership with outside agency referral sources
- Shares information with the team to develop more comprehensive services for the program
- Identifies appropriate training needs for skill development and follows through on meeting those needs.
- Continually updates and adds to resource material to stay current and timely with information provided.
- Other duties as assigned.
Qualifications
MINIMUM QUALIFICATIONS:
Master’s Degree in Psychology, Social Work, counseling or related field.
At least two (2) years experience in direct service provision in a mental health setting
The ability to work with all persons with multiple challenges (mental illness, substance abuse, physical challenges, developmental disabilities, etc.).
LICENSE/CERTIFICATION:
A valid automobile operator’s license, vehicle and state minimum require automobile coverage is required.
SKILLS:
Requires basic computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as Gmail.
