***OVERNIGHT POSITION***
Description:
This position if responsible for the overall coordination of Agency referrals for Medicare and non-Medicare clients. The LVN intake Coordinator is a resource person for referral sources and works closely with other members of the team to ensure adequate coordination and transition occurs to home health services. The intake Coordinator should be extensive knowledge of the regulatory guidelines governing home health admissions.
JOB DUTIES/ KNOWLEDGE:
1. Receive referrals from physicians, facilities, and staff and enters referrals, processes admission paperwork
2. Clinical review/appropriateness for home health and coordinating with all referral/intake information to appropriate Scheduler and Clinical Supervisor immediately once patient is ready for admission.
3. Responds promptly to referral management system referrals, i.e. Allscripts, Curaspan, and Epic Link.
4. Obtain pertinent medical information to communicate to clinical team regarding wounds, IV’s, catheters, etc.
5. Verifies Medicare and Insurance Benefits.
6. Reviews/Tracks pending referral log on a daily basis.
7. Maintains pertinent notes in EMR (Kinnser) for clinical team review to assist in care of the patient.
8. Communicates referral information to Marketing Staff.
9. Responds to telephone calls and/or visitors in a warm, sincere, and timely manner
10. Responsible for confirming PCP with following verbal orders as needed.
11. Responsible for coordinating a new PCP as needed
12. Rotating Weekends and Holiday On-call duty.
13. Performs other duties as assigned
QUALIFICATIONS:
1. High school diploma required.
2. Graduate of an approved school of professional nursing, RN or LVN licensed in the state in which he/she currently practices.
3. One-year experience in home health preferred.
4. Must be computer literate and type at least 35WPM.
5. Experience in Intake responsibilities and working with multi location agency.
6. Bilingual preferred.
