Hours and Benefits:

Requirements for the Care Manager:

· High school diploma and one year of case management/coordination experience including: assessment; intake; goal setting and establishing care plans; coordination of care and services; referrals and linkages; and crisis intervention, to people who meet the NYS DOH Health Home enrollment criteria.

· Valid NYS Driver’s License required.

· Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required.

· If applicable, meets NYS educational and experience requirements for providing HARP assessments and Health Home Plus & AOT services.

Responsibilities for the Care Manager include:

· Provide Health Home Services on site and in the field to a caseload of individuals with the goal of reducing the rate of preventable hospitalizations through linkage, connection and engagement with all necessary community-based resources and services for behavioral and physical health care.

· Coordinate the development of the Individualized Care/Service Plans/Plan of Care.

· Meet the Health Home and DOH requirements for outreach and engagement & active care management.

· Ensure and coordinate the completion of necessary assessments to identify client physical & behavioral health needs.
