Duties and Responsibility

Duties and Responsibility
- Assessment and Treatment: Conduct thorough assessments of patient wounds, which maybe chronic or acute and develop individualized treatment plans.
- Implement advanced wound care techniques, including debridement, dressing selection, skin grafts and infection control measures.
- Collaboration with Healthcare Teams: Work closely with other healthcare professionals at each facility to coordinate care and provide updates on patient progress to ensure continuity of care.
- Manage a travel schedule to visit multiple skilled nursing facilities on a regular basis.
- Ensure timely and efficient care delivery.
- Maintain accurate and comprehensive patient records, documenting all assessments, treatments, and patient/family interactions.
- Participate in quality improvement initiatives, staying updated with the latest advances in wound care and applying best practices in treatment.
- Complete documentation accurately and in a timely manner.
- Educate patients and their families on wound care, prevention, and management.
- Monitor patient progress and adjust treatment plans accordingly.
- Valid License as a Physician Assistant.
- Minimum of 1 years’ experience in advanced wound care is highly preferred.
- Certification in wound care (e.g., CWOCN, CWS, WCC, WOCN) is a plus.
- Ability to travel to multiple locations.
- Strong interpersonal skills and the ability to work independently.
- Excellent organizational and time management skills.
- Wound Expert EMR knowledge preferred.
- Frequent standing, walking, and sitting required for extended periods of time.
- Requires visual and hearing acuity.
- Able to lift a minimum of 25 pounds.
