Job Posting for Case Manager (Bilingual) at Residential Treatment Center
Shiloh is a residential treatment center that provides services to children and adolescents with psychological issues and behavioral problems. The case manager position is at the center of our interdisciplinary treatment model and coordinates care among all treatment departments. The case manager is also the liaison to the family, the contractor and other community service providers involved in the child's care.
Duties and Responsibilities:
Completes or ensures the completion of the following: Admission Assessment, Individual Service Plans, and Discharge Summaries according to Shiloh Policies and Procedures
Monitors contact between family members and clients to identify potential problems and consults with parents regarding client behavior and issues of client care
Inform clients of rights through the client handbook and general information
Maintains a comprehensive knowledge of the standards and policies of Shiloh, TDFPS, and incorporates these into work behavior, interactions with clients and in personnel matters
Maintain ongoing communication with primary caregiver to provide and obtain pertinent information
Complete thorough assessment of potential sponsors and explain ORR reunification requirements
Assist potential sponsors throughout the reunification process as needed; ensures accuracy
Complete and verify family reunification packets are accurate and current
Document all efforts and communication with primary caregivers, potential sponsors, and other interested parties
Maintain ongoing communication with outside providers to ensure services are provided in a timely manner
Keep regular communication with the treatment team to ensure any identified mental health needs are being addressed appropriately and in a timely manner
Provide weekly, in-person updates to the client and telephonic updates to the primary caregiver/sponsor
Compile, prepare, and submit an appropriate recommendation based on the client's best interest, progress, and stability
Maintain accurate records, files, forms, and additional information in accordance with agency policies, licensing and/or contract requirements
Ensure confidentiality in all program operations and service population
Participate in team meetings and ORR trainings
Willing to work evenings, weekends, and holidays as needed or requested by Lead Case Manager
Requirements:
Bachelor’s Degree in Social Work, Psychology, or other related social service field
Fluent in Spanish and English
Familiarity with mental health diagnoses and symptoms
Proficiency with writing/documentation
Strong organizational skills and attention to detail
