Wister Systems is a co-ed licensed CRR providing a therapeutic residential environment to individuals with serious and persistent mental illness. Wister residents have some history of forensic involvement.
The Resource Coordinator is an administrative position responsible for collaboratively and actively assisting the participants of Wister System with organizing resident care, assisting with development and implementation of Residential Service Plans, and working with the residents and their support teams in the development in their care in the areas of Medical and Psychiatric,etc. The Resource Coordinator assists the residents of Wister Systems attain overall wellness using a strength base clinical model for effective engagement.
Coordinate community based resources and activities for residents.
Complete state mandated documentation,i.e, Residential Service Plans, assessments, Intake and Discharge Document
Admit and Discharge Residents utilizing a Electronic Health Systems
Accompany residents on medical appointments as needed.
Meet with residents weekly to access needs.
Three years experience working with adult with Mental Health diagnosis / community outreach work experience.
Experience working in a mental health residential facility
'Prior Case Management experience
High School Diploma/GED
Valid Driver's License
Ability to utilize Electronic Health Records
Good organizational skills and ability to multi-task
Good verbal and written communication skills and a comfort level in speaking with the public
Candidate must have knowledge of health care and other related resources both locally and regionally
Candidate must be able to work flexible hours as needed

RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.
