Are you a licensed social worker looking for a great opportunity to work with patients in the comfort of their own home? Are you an independent problem solver?

Our social workers assist patients in fulfilling their needs in the community. We have Home Health patients in need of a true professional that wants to make a real difference in their lives.

Responsibilities and Duties

· Attend weekly interdisciplinary team meetings

· Perform in home visits for exploration of community resources

· Provide counseling to patients and their families

· Compile assessment data for individuals

· Complete documentation of all your hard work!

Qualifications and Skills

· Masters of Social Work (MSW) degree from a school of social work accredited by the council on Social Work Education.

· Home Health experience preferred

· Bilingual preferred
