Summary/Objective
Provides appropriate Therapeutic Behavioral Services by assisting clients and families in gaining access to necessary services intended for maximum reduction of symptoms of behavioral problems and increase in social skills. To provide assistance supervising/monitoring youth in daily activities. Services clients in the community and in-home, after school hours, with availability for school hours as needed. Outpatient Case Manager will operate on a 4 days a week schedule, with a goal of 26 client interaction units a week, with time off removed from standard as utilized.
Essential Duties and Responsibilities
· Provide clients interventions using evidence based techniques in community setting including office when applicable, in-home visits, and community based services.
· Utilization of crisis prevention methods as well as restoration of social skills and daily functioning
· Close collaboration with licensed clinician on Treatment Plan development and necessary updates
· Identify and strategize treatment options for clients based on individual needs
· Implementation of referral and linkage to other necessary healthcare providers
· Required to transport youth
· Maintain a flexible schedule consistent with the needs of the client with the expectation of 4 days a week, with availability for going into schools, community, homes and clinical office to service youth as necessary.
· Facilitates group activities and development, assists with activities appropriate for group’s level of functioning
· Assists with neutralizing negative and dangerous situations
· Maintain required direct care interaction including all necessary documenting and reporting, attendance of meetings, trainings, and all standard measurable metrics and agency standards set for role
· Maintain Client Interaction of 65% of worked hours
· Establishes and maintains positive working relationship internally and externally
· Support the agency’s effort to maximize productive activity
· Communicate with agency staff in a professional manner
· Maintains quality results by following agency standards-attend clinical supervision, manage caseload effectively, manage and maintain treatment plans, adhere to on time and quality documentation standards
· Maintains continuity of work operations by documenting and communicating actions, irregularities, and continuing needs
· Maintains patient confidence by keeping patient records, information confidential per HIPAA
· Serves and protects the agency by adhering to professional standards, agency policies and procedures, federal, state, and local requirements, and CARF standards
· Perform other duties, as assigned, to the need of the agency deemed necessary and appropriate
