Serves as the primary contact for direct services to provide community-based intervention and prevention services to individuals with intellectual disabilities and behavioral health care needs at the START Resource Center. Provide integrated care and treatment with collaboration among all team members. The service involves therapeutic respite, emergency and planned services. Position Requirements: High school diploma or GED and One year of experience, have direct support experience with individuals with mental health and intellectual disabilities who have serious behavioral challenges and experience with crisis management OR have a Bachelor’s degree in Human services field. If degree is not in Human services, must have experience with population. Valid driver’s license (this is a condition of employment) including personal vehicle insurance coverage for liability. Must maintain proficiency in company sponsored training and certifications specified and or required by future company regulations. Maintain CPR and First Aid certification. Successfully complete and maintain training courses as required or amended by company policy. Responsibilities: Provides direct support for individuals (no more than 4) with disabilities and challenging behavior in a community and/or residential setting. Assists the person(s) receiving services in identifying and communicating his or her requests and needs for services and supports. Implements services and supports to address the person’s goals and desired outcomes as identified through START Team. Seeks to understand the individuals in the context of their personal history, their social and family networks, and their hopes and dreams for the future. Provides a therapeutic environment for individuals at all times. Relies on direction and supervision to accomplish the job. Provide integrated care and treatment with collaboration among all team members. Ability to support someone with behavioral challenges and implement effective crisis management protocols. To perform this job successfully, an individual must be able to perform each essential responsibility satisfactorily. The requirements listed below are representative of the work conditions, knowledge, skill, and/or ability, and physical and sensory requirements needed to perform the essential duties. Must be able to lift a minimum of 70 lbs. Must be able to pull a minimum of 50lbs. Must be able to squat, kneel, crawl, crouch, climb, and stoop. Must be able to regularly use hands to finger, handle, or feel objects, tools, or controls. Required to regularly stand and walk. Must be able to demonstrate proficiency in CPR from the floor level requiring to work on hands, knees, bending, standing and lifting.
