Position: Care Manager (participants with children)
Accountable to: Program Director
Job Classification: Salaried exempt (e.g.)
Percent of Direct Service Time: 100%

Overall Purpose of the Position:
To provide families in our program support —in order to enhance structure and growth in their lives that will assist them in moving towards self-sufficiency. Also, to provide support to the families volunteer team. This position will manage our families in the Anderson Area. At the present time the home office for this position is in Seneca. Within the next few months this position will have a satellite office in Anderson SC.
Educational and Experiential Criteria:
- Able to communicate effectively with both the participants and Volunteers
- Able to problem solve quickly and effectively
- Flexibility and open mind to innovative ideas and activities
- Bachelor degree in related field or 2 yrs experience working in the area of family case management, client advocacy
Responsibilities:
- Daily/Monthly Responsibilities
- Give support, resources to overcome obstacles and handle resolution of participant issues
- Scheduling and giving reminders to participants and team for the 90 day team/participant meetings
- Work with participants to uncover immediate and ongoing needs
- Provide encouragement and motivation to participant
- Work with the Financial Care Manager & Children’s Care Manager for the overall completion of the Family Success Plan.
- Managing the Volunteer Staff of Mentors
- Interview and selection of mentors
- Support during course of the program
- Weekly communication with participants/mentors
- Assure weekly communication between volunteer team members and between mentors and participants
- Scheduling the quarterly Mentor meeting, with the assistance of CM of Pickens.
- Schedule and facilitate the training for mentors with CM of Pickens County, FCM and Children’s Care Manager ( as needed)
- Provide mentors with updates to supporting documentation and tools
- Monthly/End of Month Reporting
- Keep all communication records current in compliance with reporting expectations.
- The MCM is responsible for recording the following on a weekly/monthly basis
- Case Notes on participants weekly in charity tracker
- Monthly Care Manager Report to Program Director
- Participant Reward points, monthly
- Participate measurements monthly given to Admin/Impact Specialist
- Track daily activities in a timekeeper for the first 90 days of employment
- Other Duties
- Coordinate volunteers and structure for monthly group “The Well”
- Organize and schedule programs that will be stand-alone. Ex. workshops for parents
- Building and maintaining participant resource partnerships, and establish new ones
- Other duties as needed

- Saturday ( occasionally throughout the year there are Saturday events that are scheduled way in advance.)

- 5 PM
Typical start time:
- 9 AM
*There are times you will have to schedule team meetings with your participant after 5. This depends on the work schedule of the participant and their mentor/financial coach. There are also scheduled group evening events. When events/team meetings are scheduled after 5 you will be able to come in later that day or choose to leave early another day within the same pay period. This is referred to as “ Adjusted Time”

Pay: $38,000.00 - $43,000.00 per year
