RISE Services (www.risesgroup.com) is a human service provider looking for dedicated employees. We are looking to fill a few Pre-ETS Transition Coach positions. This is great for teachers, bus drivers, or overnight shift work. These positions may provide small group classes utilizing our curriculum already approved by DORS. This position also is for individuals who will need short term work experiences. At times, we will find the location for these work sites and others we will just provide the supports on site during this short work experience. Most take place during the summer or school breaks; however, there are times when we receive referrals with specific requests for outside of school breaks.

DARS works with students, their families, their schools and community partners to enrich transition planning and support students with gaining knowledge and experiences necessary so they may make informed decisions about their future. Pre-ETS Transition Coach:
Seeking an experienced professional to join our team as a Pre-Employment Transition Services (Pre-ETS) Transition Coach. We are looking for an organized self-starter that is passionate about improving the lives of high school students with disabilities that are transitioning into adulthood. The perfect opportunity for a retired Teacher, Job Counselor, or Social Worker; the Pre-ETS Transition Coach will guide students individually, as well as in a group setting on workplace readiness, job exploration, and work-based learning.
Due to the flexibility of this position, a great deal of independent judgment is required to plan, prioritize, and organize the diversified workload which includes individual and group instruction and teaching, completion of primary state-required forms, infrequent meetings, curriculum planning, and parent and student interaction. Teaching hours will be in person, but prep and paperwork hours can be done remotely. Must have access to a laptop for services.
Essential functions/ duties:
- conduct engaging, relevant, informative, and dynamic group sessions with high school students with disabilities, leading activities that cover the range of pre- employment transition services
- Lead outreach and recruitment efforts for attendees for new groups throughout the year, including disseminating outreach Flyers; holding information sessions; And answering questions as needed from referring partners, interest participants, and family members.
- Develop and maintain positive relationships with partner agencies, school personnel, businesses, educational institutions, other departments and/ or vendors to enhance menu of services, continuum of care, and the number of employment transition opportunities.
- Provide general case management, maintaining excellent communication with participants, family members, funding and referring agencies, schools and other partners; Submitting accurate and detailed monthly reports on time; And assisting with the billing process. Includes communicating with funding and referring agencies regarding the direction of services; Attending and leading all pre- ETS group meetings as scheduled; scheduling 1-ON-1 appointments with participants as needed; And attending team meetings.

- manage own schedule and accurately document time spent with each individual.
- Be aware of and respond accordingly to any specific needs of the participants in the groups, including mental health safety, accommodations needed in a group setting, etcetera.
- Conduct tours and informational interviews with representatives from employers and educational programs to choose the most relevant and highest quality settings for participation in pre employment transition group activities.
- Provide consultation, guidance, and disability awareness training (as needed) to employers and educational program representatives to foster positive partnerships.
- Complete in an accurate and timely manner all monthly billing reports, participant reports, case notes, and other supporting documentation, meeting utilization expectations.
- Attend relevant funding and referring partner meetings to discuss participants, receive feedback and updates, and market services with service source to increase number of referrals.
- Coordinate and participate in support team process and other meetings pertaining to participants individual support plan.
- Prepare annual individual support plans as needed.
- Transport participants on hours of employment and educational settings.
- Ability to travel to multiple locations, at times with little notice required.
- Ability to work flexible hours, when needed, including weekends and evenings required.

- Requires knowledge of computers, Microsoft Office suite, Internet and e-mail navigation with the ability to demonstrate and teach these skills to participants during job development and job coaching
Qualifications:
- Education:
- High school diploma (required)
- Associate degree (Preferred)
- Experience:
- One year of experience teaching in public or private schools, and forward or providing instruction to individuals with disabilities (preferred)
- Workforce development: one year (preferred)
- Youth Services or related field: one year (preferred)
- License:
- provide proof of current driver's license, auto insurance, and access to own transportation (Required)
