River Edge Behavioral Health is seeking a full-time Case Manager for our Assertive Community Treatment (ACT) program in Milledgeville, GA. Duties and Responsibilities:
- Implements and organizes the delivery of specific social services within the community.
- Monitors clients' progress toward treatment goals.
- Assists clients in locating and utilizing community resources including legal, medical, and financial assistance.
- Provides referral services to those organizations as needed.
- Provides resource linkage and skills training to clients living in personal care homes and transitional housing; assists in pre-release and release planning.
- Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
- Implements skills training for recovery management, activities of daily living, and social/interpersonal skills as needed.
- Prepares monthly case reviews of progress toward treatment goals.
- Provide transportation for clients as needed.
- Other assigned duties.
Qualifications:
- Bachelor’s degree in social service field (Social Work, Sociology, Psychology, Human Services) or related field.
- Must have a valid GA Driver’s License.
- At least 1-2 years of providing behavioral health service.
- All applicants must pass a satisfactory background clearance and pre-employment drug test.
- Provide 2 Professional references (Former employers only, no family members)
- Demonstrate expertise and knowledge in mental health and substance abuse services and community resources.
- Exemplify excellent communication skills, both verbal and written.
